Applicable if and when the Longevity Center operates as a HIPAA covered entity or as part of an organized health care arrangement.
Required sections
- The header and content required verbatim by 45 CFR 164.520, in the required format
- Uses and disclosures of protected health information permitted without authorization
- Uses and disclosures requiring written authorization, and how authorization is revoked
- Individual rights: access, amendment, accounting of disclosures, restrictions, confidential communications, and a paper copy
- The covered entity's legal duties, including breach notification
- How to file a complaint with the entity and with the U.S. Department of Health and Human Services
- The privacy officer contact and the notice's effective date
